To obtain your authenticated driver record from the Ohio Bureau of Motor Vehicles send a subpoena duces tecum to: Ohio BMV, P.O. Box 16583, Columbus, Ohio, 43216. You can accomplish the same result by filing a records request filing BMV form 1173.
